- Two (2) Exams

Adult Periodontal Plan $549
- Two (2) Exams
- Two (2) Adult Hygiene Therapy

Child Plan $349
- Two (2) Exams
- Two (2) child hygiene therapy

- 15% off regular fees for other dental treatment
Payment must be made in full for each family member prior to the 1st dental wellness visit. Both dental wellness visits must be made in the 12 month consecutive period or the offer becomes VOID and the benefit is lost. The offer cannot be combined with any other dental plan. The In Office Dental Plan includes 15% off our regular fee for Fillings, Root Canals, Crowns, Implants, Periodontal Treatment & Oral Surgery.
The In Office Dental Plan DOES NOT include Orthodontics.
Payment is due at the time of service for Cosmetic and Restorative Treatment. It is the patients sole responsibility to schedule and keep their reserved appointments.
